Korelasi Kadar Serum Basal AMH dengan Respons Ovarium terhadap Stimulasi Ovulasi pada Program Fertilisasi In Vitro
ABSTRACT: Anti-Mullerian
Hormone (AMH), produced by pre-antral and antral follicular granulosa cells, has
a role in limiting primordial follicular number to develop into primary
follicles. Its level can be measured in the serum, which is relatively not
fluctuating along the menstrual cycle and its level is decreasing gradually
along with the increasing age of a woman. The number of developing follicles
depends on the number of remaining follicles in the ovary and the numbers of
mature follicles. The number of oocytes obtained in ovarian stimulation depends
on the developing follicular number. Therefore, the examination of serum basal
AMH level can indirectly describe ovarian reserve and predict ovarian response
on ovulatory stimulation. The objective of this study was to examine
correlation of basal serum AMH level and ovarian response with ovulatory
stimulation in in vitro fertilization program. Population was women who met the
inclusion criteria to participate the in vitro fertilization program in
Fertility Clinic, Siloam Hospital, Surabaya, and Graha Amerta, Surabaya, from
Februari 2007. Samples comprised 69 persons. AMH level was examined on day 3
menstruation along with the examination of other basal hormones. Data were
collected during stimulation process up to ovarian harvesting procedure, which
included total gonadothropin ampules, serum E2 level during hCG, pre-ovulatory
folicular count, and oocyte count. Statistical analysis was undertaken using
Pearson’s correlation test to identify intervariable correlation. The result of
this study revealed significant correlation between basal serum AMH level and pre-ovulatory
follicular count (r = 0.529, p < 0,01), oocyte count (r = 0.535, p <
0.01), serum E2 level during hCG administration (r = 0.456, p < 0.01), and
total gonadothropin ampules per pre-ovulatory follicle (r = -0.311, p <
0.01).
